Equipment guidance
What to have ready when requesting equipment
A practical starting point for your first equipment conversation, whether you have an order or need help beginning.
Start with the equipment you want to discuss
If you have a doctor’s order, keep the equipment name and any fitting instructions nearby. If you do not have an order, you can still ask how to begin. Your treating provider determines whether equipment is appropriate and whether to write an order.
- The equipment name on your order, if you have one
- Your ordering provider and any fitting instructions
- Your location and questions about next steps
Begin with a phone request
Call 844-407-4440 for new equipment requests. Requests can be taken 24/7: Avery connects you with an equipment-trained AI agent when one is available, or collects your equipment request, callback number and preferred time. Documentation, availability, benefits and any patient responsibility still require review; a call is not an insurance approval or delivery promise.
When Avery answers, tell her the equipment name you selected.
Keep documents in approved secure channels
Ask the team how to send documents through an approved secure channel before sending them. Do not put patient medical information, prescriptions, insurance details or documents into the general contact form. That form is for general contact, not equipment orders or insurance qualification.
Choose the right pathway
For an existing request or equipment service, visit Patient support to reach Patient Experience. Referring providers can use OHC’s Parachute Health supplier page.
Complex Rehab services are available in Texas only.
